By applying to register to participate in competitions, activities or events of the Austral Volleyball Club, you (and, if you are under 18, you and your parent or legal guardian whose name and details appear in the requisite fields) acknowledge and agree that your application for membership will be considered and may be accepted or rejected in accordance with the Club's Terms of Registration. If the Club accepts your application, you (and, if you are under 18, you and your parent or legal guardian whose name and details appear in the requisite fields) acknowledge and:

• Agree to abide by Club policies.
• Understand the Club will collect member details for the purpose of providing member services and adopts the Volleyball Australia Privacy Policy in relation to the collection and storage of personal information.
• Understand that as a member of this Club, I will be asked to contribute and volunteer to assist in activities such as coaching, fundraising and other Club activities from time to time to help make our Club the best we can be.
• Agree that the player listed herein is fit to play competitive volleyball understanding the inherent risks.
• Give permission for the medical information that has been provided on this form to be made available to the team coach, manager or relevant responsible adult for the purpose of attending matches.
• Agree to pay the Club membership fees in line with the team or squad I have been selected for when due. Alternatively, I will arrange a payment plan with the Committee in line with the Club's requirements and agree to make payment of the full amount when due.
